Commit and tag from the repository root

There is no .git in packages/mcp-server, so the commit and tag were skipped without complaint: 0.2.3 went to npm while main still said 0.2.2, and gh release tagged the pre-bump commit.

The release now bumps with --no-git-tag-version, reads the version back off disk, and commits and tags from the root. 0.2.4 carries no functional change — it is the first release cut correctly by that path.
